Living with a chronic or serious illness can be like running a marathon—it takes courage, determination, and friends to cheer you on. The Burke Hospice and Palliative Care Transitions program can help you find the endurance and strength to make it through each day's race.

Your Own Cheering Section

Our Transitions program helps people with serious illnesses find the support and resources they need to live more comfortably and easily. More simply—your Transitions Coordinator (a social worker or nurse) and Volunteer are new friends who understand your struggle with the health care system, tough treatments, and ever-changing family roles and responsibilities.

   
 

Burke Hospice Transitions

The Transitions Coordinator and Volunteer will schedule regular visits and phone calls to talk about your needs, fears, hopes, and plans. He or she will…

  • Provide emotional support, companionship, and conversation
  • Offer your family members caregiving tips and suggestions for stress relief
  • Refer you to local resources to ease your medical, social, emotional, or financial situation
  • Help with errands or small tasks
  • Give caregivers a "rest break" for shopping, going to appointments, or just to relax
Free Support, Priceless Care

The Burke Hospice and Palliative Care Transitions program is free to anyone who has a serious illness. We offer Transitions as a community service for residents of Burke, Catawba, Lincoln, Cleveland, Rutherford, McDowell, Avery, or Caldwell counties.
