Your Own Cheering SectionOur Transitions program helps people with serious illnesses find the support and resources they need to live more comfortably and easily. More simply—your Transitions Coordinator (a social worker or nurse) and Volunteer are new friends who understand your struggle with the health care system, tough treatments, and ever-changing family roles and responsibilities. |

The Burke Hospice and Palliative Care Transitions program is free to anyone who has a serious illness. We offer Transitions as a community service for residents of Burke, Catawba, Lincoln, Cleveland, Rutherford, McDowell, Avery, or Caldwell counties.
